Method 1: First Offline Method to Fill Out the SBI ATM Form
Different ATM forms can be found in different SBI branches. If you have the SBI ATM Application Form as shown in the below image, then you can fill it out by following the steps given below.

Step1: First of all you have to write the address of SBI Bank as we have written on the top right side of this forum.
Step2: Then after that, you have to write your name and full address
Step3: The bank’s account number must be written down at the end, followed by your signature and the date.

Step1: First of all you have to write your name and full address and then enter your mobile number and email id.

Step1: On the top left side you have to write the name of the branch and on the top right, enter your account number and date.
Step2: You are seeing several options in this form because you can do many things at once like taking a checkbook, doing corrections to the mobile number, etc.
We have to apply for a new ATM card, so we will choose option 8 as shown in the above photo.
Step3: Then after this, we will fill in our address. If your old and new address is the same, then fill in the same address in both places.
Step4: After writing your signature, mobile number, and name, your form will be complete.

Method 4: Online Method to Fill Out the SBI ATM Application Form

Step1: First of all, you have to open your SBI Net Banking. To open SBI Net Banking, you have to go to the official website of SBI. After filling in your User Name, Password, and Captcha Code, click on the Login button.
Step2: After clicking on the login button, a new tab with an OTP filling option will open.
When I log into SBI Net Banking, an OTP is sent to our mobile device because I have turned on 2-Step Verification on our account.
After filling it out, we will click on the “Submit” button.
Step3: After successful login, an “e-Services” option will appear on your screen. We have to click on this option, we have shown the same in the photo below.
Step4: Now a new screen will open in front of you, in this you have to click on the option of “ATM Card Services”.
Step5: On the next screen, you will see the option of “Request/Track Debit Card”. We have to click on this option.
Step6: Now on the new window, first you have to select your account number and then the category of your card.
In the “Name on the Card” option, you have to write your name, whatever name you want to be printed on your debit card.
In the “Select Type of the Card” option, you have to select the debit card you want to order.
After clicking on the button of Term and Condition, you have to click on the “Submit” button.
Step7: Now we have to verify our address and then click on submit button.
Step8: Now two options will appear on our screen.
- Using One Time Password (OTP)
- Using Profile Password
Through both these options, we are being asked by which method we want to do the verification. Here, we have selected the option of “Using One Time Password (OTP)”.
As we click on this button, an OTP will come on our mobile number.
Step9: As soon as we click on the submit button after filling in the OTP, our debit card application will be completed and after some time the SBI debit card is sent to our home address.
