0. Bank and Account Related Information
Date
First of all, write the date on which you are filling out this KYC form.
Branch Name
Write the name of the branch, where you are going to submit this SBI KYC Annexure B.
Branch Code
Enter the SBI branch code number where you want to submit this KYC form. You can find this code on your SBI bank passbook or online.
Customer ID
Customer ID can be obtained from the first page of your passbook.
Account Number
You should enter the account number of your SBI savings account or current account. The account number is also available on the first page of the passbook.
CKYC No.
You must fill out the CKYC number; SBI staff can help you find it.

You can skip this point.
Name
You should write your name in block letters. Don’t forget to have a space in your first and last name.

Date of Birth
Here write your date of birth in DD/MM/YYYY format.
Gender
Here, you specified your sex. For instance, select “Male” if you’re a man.
Martial Status
In this case, you must check the corresponding box: check the “Married box” if you are married, and select the “Unmarried” box otherwise.
Name of Father/Mother
Here you have to write the name of your father/mother/wife.
No. of Dependent
This is an option that you can skip. Please enter the number of family members who are reliant on your income.
ILLITERATE
In this field, you are being asked if you are educated or not.
Name of Guardian (In Case of Minor)
If you are filling out this form for the KYC of a minor (who has not yet turned eighteen), then the names of the parents should be entered in this column. If the applicant is not a minor, you can skip this field.
Nationality
If you are an Indian, tick the “IN-India” option otherwise enter the name of the country in which you reside.
Occupation Type
Through this option, you are being asked what type of work you do. For example, you can select the “Government Sector” if you do a job in a government department. If you are a woman who performs house chores then you can select the “House Wife” option.
Monthly Income
Enter your monthly income in this field.
Net Worth
Provide a rough estimate of your total assets.
Source of Income
In this section, you have to describe from which sector you earn your income. For instance, you can choose the “Agriculture” if you are a farmer.
Religion
You have to select the religion you follow.
Category
Click the appropriate box according to your caste.
Person with Disability
If you are a DIVYANG, you can choose the “Yes” box otherwise select “Not”.
Educational Qualification
Check the box that best describes your level of education.
Organization’s Name
You can skip this option.

Citizenship

Country of Tax Residence in India Only

2. Contact Details
Enter your Mobile Number and Email ID in the respective fields.
3. Proof of Identity
You must select the box corresponding to the document you are adding to the application, such as a PAN card or an Aadhar card. We advise you to tick the “D-UID(AADHAR)” option.
Documents Number
Enter the 12-digit Aadhar Number if you tick the fourth option which is D-UID(AADHAR).
Issue Date/Expiry Date
If you choose the “D-UID(AADHAR)” option, then you can skip this option otherwise you have to enter the “Issue and Expiry Date” of the documents you are attaching with this application.
4. Address Details (Current)
You have to fill in your full address in details like your Address type, House number, Landmark, City/Village, District, State, and PIN Code.
5. Address Details (Permanent)
If your permanent address is the same as that of your current address, you can tick the “Same as Current/Permanent Address” option. OR if your permanent address is different then you can fill these fields.
6. Proof of Address
Select the appropriate box for providing documents related to your address, which you are providing with this application.
You can tick the “Utility Bill” option if you are attaching an electricity bill with this application.
Document Number
Write the document number which you are attaching with this application for address proof.
Date
Enter the date mentioned on the Address Proof Document.
7. Declaration
In this section, you have to accept all terms and conditions by ticking the “YES” option. After accepting all terms and conditions, affix your passport-size photograph in the required field and then sign the application.

Enter the place from where you are filling out this application form.
