Ayushman Card is a health card that is issued to the beneficiaries of the Ayushman Bharat Yojana to avail of the benefits of the scheme.

The name and other important information of the beneficiary are printed on this card, which is used during the treatment of the cardholder.

1. Check Ayushman Bharat Eligibility Through Its Website

The first method to check Ayushman Card Eligibility is through PMJAY official website. Follow the below steps for Ayushman Bharat Eligibility.

Step 1: Go to the official website of PMJAY i.e pmjay.gov.in and then click on the “Am I Eligible” button as shown in the below picture.

Step 2: Now enter your mobile number and the CAPTCHA code in the appropriate fields, and then click the “Generate OTP” button.

Step 3: An OTP will be sent to your mobile number, enter the OTP, and after accepting the term and conditions click on the “Submit Button”.

Step 4: Choose your state from the menu.

Step 4: The screen will now provide a new option called Select Category. Choose the category by which you would like to find the Ayushman Bharat Eligibility.

You can search for your Ayushman Card Eligibility in the Select Category option by selecting your Name, Household Number (HHD No.), Ration Card Number, and Mobile Number.

For this example, we’ve opted for the “Search By Name” option.

Step 5: After selecting the “Search By Name” option, a new form will come out on the screen as shown in the below image.

In this form, you don’t need to enter all the details, just enter some important details to find Ayushman Card Eligibility.

For your kind information, we have used the beneficiary’s name, gender, rural or urban region, and the PIN code of the neighborhood where the beneficiary resides, to determine the Ayushman Card Eligibility.

Step 6: Enter the details of the person whose you want to find the Ayushman Bharat Eligibility, and then click on the “Search” button.

Step 7: As soon as, you click on the “Search” button, a list of the entitled beneficiaries will pop out on the screen.

2. Check Ayushman Card Eligibility Through PMJAY Mobile App

The second method to check Ayushman Bharat Eligibility is through PMJAY App. Follow the below steps for Ayushman Card Eligibility.

Step 1: Download PMJAY App from the Google Play Store, then after downloading the PMJAY Mobile App, click on the “Open” button.

Step 2: Click on the “Check Eligibility” button as shown in the below picture.

Step 3: Select the state from the menu.

Step 4: After selecting the state, a new option “Select Category” will pop out on the screen.

Step 5: When you click on the “Select Category” button, various options will emerge on the screen, as shown in the below image. We have selected the “Search By Mobile Number” option to find Ayushman Card Eligibility.

Step 6: Enter the mobile number of the person whose you want to find the Ayushman Bharat Eligibility and then click on the “Search” button.

Step 7: Now a list of all the family members will emerge on the screen containing the important database of all the family members.

3. Check Ayushman Bharat Eligible Beneficiary Name by Contacting PMJAY Customer Care

The third method to find Ayushman Bharat Yojana Eligibility is by contacting PMJAY Customer Care.

You can call Ayushman Bharat Yojana Customer Care and ask them that Am I eligible for this scheme or not?  After some scrutiny, PMJAY Customer Care will tell you whether you are eligible or not for this scheme.

The fourth method to check Ayushman card Eligible beneficiary name is by visiting to your nearest empanelled public or private hospital.

Go to the nearest empanelled public or private hospital and ask about your eligibility from the Pradhan Mantri Jan Arogya Mitra (PMAM).

What is PMAM: Pradhan Mantri Ayushman Mitras are individuals who are appointed at all empanelled public or private hospitals to assist Ayushman Card Holders in the hospitalization process.

If you don’t know the address of your nearest empanelled public or private hospitals, follow the below steps.

Step 1: Download PMJAY App from the Google Play Store, then after downloading the PMJAY Mobile App, click on the “Open” button.

Step 2: Click on the “Find Empanelled Hospital” button as shown in the below picture.

Step 3: Now choose appropriate options in the given fields and then click on the “Search” button.

Now the list of empanelled hospitals nearest to you will come out on the screen.

Ayushman Card Eligibility Criteria

Beneficiaries of the Ayushman Bharat Yojana have been identified using data from the 2011 Socio-Economic Caste Census (SECC).

The PMJAY Scheme also includes families that were a part of the Rashtriya Swasthya Bima Yojana (RSBY) but were not present in the SECC 2011 database.
